How to get started

enrolling in Head Start...


We are accepting applications all year long, January through December. Our school year runs from about the third week of September through the third week of May. A child must turn three before September 1st to qualify for the present school year. We serve 572 children at 6 different locations throughout Clackamas County. Children are not selected on a first come, first serve basis. We select children with the highest need and the lowest income. View this years Federal Poverty Guidelines to see if you qualify. We are permitted to enroll a very small percentage of over-income families but we must reserve these openings for the families that are in high need.

It is very important that we receive all the information requested. Fill out the Pre-Application, Adult Education Survey, Dental History, Health History, and Health Appraisal form. Please mail or fax these back with a copy of your child’s immunization record, a copy of your W-2(s) and a copy of your last pay stub. We must verify income for all parents who have lived in the home in the past 12 months. Fill out the top section of the Health Appraisal form and the Dental History form and take those in to your child’s Doctor and Dentist to have them write in the date of the last examination and sign it. You may want to ask the Doctor or Dentist to simply fax (503-675-3551) the form to us once they have completed it.

What is Head Start?/¿Qué es Head Start?    This form tells a little about Head Start as well as giving you a list of some of the information we need sent in.

Application/Aplicación: Fill out this form as completely as possible. Please be sure to sign the bottom

Child Health History/Historia de Salud del Nino(a): Fill this out completely. Foster parents: Fill out what you know.

Adult Education and Training Survey/Encuesta de la Educación de los Adultos: This is to be filled out for the persons having custody of the child. If you are the parent, relative, or foster parent, fill this out for your education level.

Child’s Dental History and Exam Form/Historia Dental del Nino(a) y Forma para Examen Dental: Parents, fill out section 1 completely. Make sure when the doctor fills out section 2 that he/she signs it, puts in their address and date of exam and either returns it to you or faxes it to us.

Child’s Health Appraisal Form/Valoracion de Salud del Nino(a): Parents fill out section 1 completely. Make sure when the doctor fills out section 2 that he/she signs it, puts in their address and date of exam and either returns it to you or faxes it to us.

Want to get involved?

Just a few ways to get involved in Head Start:

  • Donate - Money, clothes, classroom supplies, toys, paper, computers, office supplies, etc.
  • Volunteer - In the classroom, the office, the kitchen, special projects, and more.
  • Join Policy Council - a group of parents and community members that are elected at center level to participate in decision and policy making of the Head Start program. This group of elected members consists of at least 51 % parents of the children currently enrolled in Head Start and the council consists of interested community members.
  • Feedback - please provide us with your input.
